Do not clean visible mold with bleach - it doesn't kill the root structure and water from cleaning makes it worse.

Mold coverage is highly variable. Most policies have a mold sub-limit of $1,000-10,000. Mold caused by a sudden water event (covered loss) is more often covered than mold caused by gradual leakage or humidity. Common signs: musty odor that doesn't go away, water stains on ceilings/walls, recurrent allergic symptoms (worse at home), visible dark staining at wall bases or in attic. If you're renovating, any opened wall cavity should be inspected for mold before closing it back up.
Stachybotrys chartarum (black mold) produces mycotoxins that can cause respiratory and neurological symptoms in some people. But "black mold" colloquially refers to ANY dark-colored mold, most of which are different species. Health risk depends on species, exposure duration, and individual sensitivity.
Bleach is a surface-only treatment. It doesn't penetrate porous materials where mold roots grow. Worse - the water in bleach actually feeds remaining mold. Proper remediation requires removal of porous materials, not surface treatment.
For health-sensitive occupants: yes. For real estate transactions: usually yes. For a small isolated remediation in your own home: optional but strongly recommended. Independent testing is the only way to verify remediation worked.
